The Outcomes Star for adults self-managing health conditions

The Well-being Star™ has been designed for people living with a long-term health condition, to support and measure their progress in living as well as they can. It can work as a stand-alone tool, or as part of Personal Health Plan materials. The Well-being Star™ is designed to either be self-completed by a patient, or ideally completed by a patient and health professional together.

The Recovery Star Secure™ is specifically designed for secure services, covering key areas such as quality of life, mental and physical well-being, as well as addiction and substance use.
The Outcomes Star for children

My Star™ is suitable for children in families that are identified as vulnerable/troubled and receiving services, as well as children looked after by foster carers or in a children’s home.
The Outcomes Star for people caring for others

The Carers Star™ has been developed for use with a wide range of carers, both full time and part time. It measures carer well-being across areas such as self-care, finances and overall health.
